In recognition of Lori Chow’s contribution to the work of health promotion in the Province of Ontario, Health Promotion Ontario is proud to announce the Lori Chow Memorial Award. The award is a non-monetary honour, and is accompanied by a certificate of recognition.
The award is presented annually to someone who may or may not work/serve in a specifically defined health promotion role, who;
Champions the importance of the social determinants of health
Is a strong advocate for health promotion, population and public health
Demonstrates leadership in health promotion at the local, regional or provincial level(s)
Shares her/his passion for health promotion through mentoring, and/or teaching
Nominations will be accepted and reviewed by a nominations committee who will recommend a recipient to the executive committee of HPO. The award consists of a certificate of award, and complimentary one-year membership to HPO. Please submit the following nomination/application form.
